CCN Health Adds Abbott FreeStyle Libre 3 Continuous Glucose Monitoring
We're integrated with Abbott's FreeStyle Libre 3, the world's most-used continuous glucose monitor. The integration connects FreeStyle Libre 3 sensor data to CCN Health's clinical platform via Abbott's LibreView cloud API, enabling diabetes RPM programs to receive 1,440 glucose readings per day without manual patient interaction.
LibreView Cloud Integration
Data flows from the FreeStyle Libre 3 sensor through four stages:
- Sensor measurement — A subcutaneous filament measures interstitial glucose every 60 seconds
- Bluetooth relay — Readings transmit via Bluetooth Low Energy to the FreeStyle Libre app on the patient's smartphone
- LibreView sync — The app uploads data to Abbott's LibreView cloud platform in real time
- CCN Health ingestion — CCN Health pulls glucose data from the LibreView API, processes time-in-range metrics, generates clinical alerts, and posts summaries to the patient's EHR
This cloud-to-cloud architecture means no additional hardware, gateways, or patient-side setup is required beyond the sensor and smartphone app that patients already use.
Why FreeStyle Libre 3
FreeStyle Libre 3 is the most-used CGM globally with over 5 million users across 60+ countries. For RPM programs, its specifications address the two most common operational challenges in diabetes monitoring: patient compliance and data sufficiency.
The sensor's 14-day wear period — the longest of any iCGM device — reduces sensor replacement frequency and the patient outreach burden that comes with it. Its factory calibration eliminates fingerstick calibration requirements, removing a step that causes friction in elderly and memory care populations. And with readings every 60 seconds (versus the typical 5-minute interval), the clinical data density supports both standard RPM billing compliance and advanced time-in-range analytics.
CGM Portfolio Expansion
FreeStyle Libre 3 joins Dexcom G7 and Dexcom G6 in CCN Health's CGM device portfolio. All three devices are supported across every EHR integration (PointClickCare, ALIS, Epic, athenahealth, MatrixCare, Charm Health, Ethizo, August Health) and every Medicare program where CGM data applies (RPM, CCM, PCM).
Clinicians and program administrators can mix CGM brands within a single patient population — the platform normalizes glucose data from both Dexcom's Clarity API and Abbott's LibreView API into a unified clinical view.

FreeStyle Libre 3 integration is available now to all CCN Health customers. No additional configuration or contract changes are required.
